We the undersigned being desirous to be constituted into a Missionary Baptist Church have been called together by Elders Martin and Lewis Ball who as a Presbytery have this day constituted us into a church espouse the generally received principles of the Baptist Denomination:

After the letters were ____ and read by the Presbytery Elder Martin Ball pronounced us a church and the dedication prayer was offered by Elder Lewis Ball.  After this a hymn was sung and the right hand of Fellowship was extended by the Presbytery and by each other.
 
CERTIFICATE
This is to certify that we met the above named Brothers and Sisters on the day and date specified and forming a Presbytery constituted them into a church as witness on hands and seals
