The names listed below were copied from the index of the "Confederate Pensions 1892-1895" book located in the Chancery Clerk's office in the Winston County Courthouse in Louisville, Mississippi.

The instructions at the top of each application gave this information: "No person who has heretofore received a Pension from the State can now do so without making a new Application under this form. This Application must be filed with the Chancery Clerk on or before the first Monday of September."

Form No. 1 was the "Application for indigent Soldier or Sailor of the late Confederacy, under Chapter 101, Annotated Code, 1892, and 'An Act for the relief of certain officers, soldiers, sailors, widows, and servants of the late war between the States,' approved February 19, 1892." The following questions were asked to pension applicants on Form No. 1:

What is your name?
How long have you been a bona fide resident of Mississippi?
In what County do you reside? What is the name of your post-office?
In what State did you reside when you enlisted in the service of the Confederate States?
What was the date of your enlistment?
What was the name or designation of the company and regiment or vessel in which you served?
How long were you in actual service of the Confederacy?
What were the names of the commanding officers of the company and regiment or vessel at the time you enlisted in the service?
Were you honorably discharged? When were you discharged? Where were you discharged?
If you served until the close of the war, where were you discharged?
What is your age? Are you married or single?
Do you apply for a pension because you are indigent and unable to earn a support by your own labor?
Do you hold any State, United States, County, or City office from which you are receiving, as salary or fees, the sum of $300 per annum?
Are you worth in your own right, or in the right of your wife, property at its assessed value for taxation to the amount of $500?

Form No. 2 was the "Application for indigent Widow of deceased Soldier or Sailor of the Confederacy, under Chapter 101, Annotated Code 1892, and 'An Act for the relief of certain officers, soldiers, sailors, widows, and servants of the late war between the States,' approved July 19, 1892." Do you hold any State, United States, County, or City office from which you are receiving, as salary or fees, the sum of $300 per annum?
Are you worth in your own right, or in the right of any other person, property at its assessed value for taxation to the amount of $500?
